Transportation Consent *

I give permission for my child to travel with Vision Sports staff and approved volunteers using public transportation, including BART and local transit, to and from Sunday Funday events. I understand that Athletes will travel as a supervised group, follow a structured itinerary, and be accompanied by Vision Sports representatives at all times. All reasonable safety and conduct measures will be in place throughout the trip.

I acknowledge that I have been informed these events may involve travel via BART or other forms of public transportation, and I consent to my child's participation in those travel arrangements, if needed.

Liability + Behavior Consent *
